Terms of Service
Last updated: October 8, 2026
Agreement to these terms
These Terms of Service are a binding agreement between you and ClaimClear, LLC, doing business as Verity Claims ("Verity Claims™," "we," "us"). They govern your use of the Verity Claims™ service.
How you accept. You accept these terms when you check the box agreeing to them as you create an account, and when you make a purchase. If you do not agree, do not use Verity Claims™.
These terms include a binding arbitration agreement and a class-action waiver. They are in the "Dispute resolution and arbitration" section below. You can opt out of arbitration within 30 days, as that section explains.
Our Privacy Policy and Consumer Health Data Privacy Policy explain how we handle your information.
What Verity Claims™ is, and is not
Verity Claims™ is an administrative billing-dispute tool for consumers. It reads your medical bill and the statement from your insurance (called an Explanation of Benefits, or EOB), points out possible billing errors, and helps you prepare dispute letters that you review, approve, and send.
Verity Claims™ is not a law firm and does not provide legal advice. Using Verity Claims™ does not create an attorney-client relationship. Verity Claims™ is not an insurer. It does not provide medical, tax, or financial advice.
Verity Claims™ does not determine your rights, your coverage, or what you owe. Those decisions rest with your provider, your insurer, and, where it applies, a court or regulator.
Verity Claims™ does not contact providers or insurers on your behalf, except to print and mail a letter that you approved and authorized us to mail. We do not negotiate or settle a bill for you. You send your dispute as yourself.
For legal, medical, tax, or financial decisions, consult a qualified professional.
How Verity Claims™ uses AI
What AI does. Verity Claims™ uses artificial intelligence to read your uploaded documents into line items, to analyze the notes you type for possible issues, and to generate Copilot guidance. Dispute letters are put together from fixed wording based on your audit results. Billing-rule checks, and the comparison of your bill with your EOB, are performed by Verity Claims™ software, not the AI model. The Privacy Policy explains what is sent to our AI provider, including that uploaded documents are sent in full.
AI may be wrong. AI output can be incomplete or inaccurate. It can misread a document, miss an error, or flag something that is correct.
You review and approve every letter. You must review each letter for accuracy, including every fact, date, and amount, and approve it before it can be downloaded or mailed. Nothing is sent without your approval. Letters carry a label saying they were prepared with AI assistance.
Copilot gives general information. Copilot helps you organize information and draft questions or responses for your review. It does not determine your rights or give legal advice. Take substantive legal questions to a qualified professional.
Your part. Do not submit information you are not authorized to provide.
Eligibility
You must be at least 18 years old and located in the United States to use Verity Claims™. At launch, you may only upload your own bill: a bill for care you received. Verity Claims™ cannot yet process a bill for a child, a spouse, or anyone else.
Your account
You are responsible for keeping your sign-in details secure and for activity under your account. Tell us promptly at support@verityclaims.co if you believe someone has used your account without your permission.
Acceptable use
You agree not to misuse the service. That means: no uploading documents you have no right to submit; no false statements in the upload confirmation; no attempts to break, overload, reverse-engineer, or scrape the service; no getting around limits or security checks; and no use of Verity Claims™ for an unlawful purpose or to send false or harassing letters.
Your content and the license you grant
You keep ownership of the documents you upload and the content you create, including your letters.
You grant us permission to store, copy, process, and transmit your content, including to the service providers named in our Privacy Policy, solely to provide the service to you. That permission ends when your content is deleted, except for the limited records our Privacy Policy says are kept.
You confirm that you have the right to submit what you upload.
Results and estimates: no guarantee
Verity Claims™'s findings are possible errors based on the documents you provide. We do not guarantee that every error will be found, that a flagged item is wrong, or that any result is accurate.
We do not guarantee savings, refunds, or any outcome. Recovery estimates, likelihood figures, ranges, scores, and similar outputs are estimates, not promises. Example figures shown on the site are illustrations. Your provider or insurer may reject your dispute. You are responsible for reviewing results before you act on them.
Fees, subscriptions, and cancellation
Scan. The scan is free.
Evidence. $99 for one complete audit. A complete audit is one bill and the EOB that goes with it, reviewed in full: reconciliation, findings, calculations, and dispute materials where the documents support them, with certified mailing included. This is a one-time charge. It does not renew.
Family. $59 per month or $590 per year. Family is a subscription. It renews automatically at the price shown, each month or each year, until you cancel. Family includes 2 complete audits each month, saved audit history, and dispute letters sent by certified mail after you approve them. For a monthly subscription, the month is your billing period; for a yearly subscription, it is the calendar month. Audits you do not use in a month do not carry over. Beyond the included audits, an additional audit is $39, a one-time charge available to active Family members. The free scan is not an audit and does not count. If you join Family within 30 days of an Evidence purchase, the amount you paid for Evidence is credited toward Family.
Household. Family covers your household: you, and the people whose medical bills you are responsible for and are entitled to handle, such as a spouse or partner and children or other dependants in your care. Their bills are uploaded and handled under your account. You confirm that you are allowed to share each document you upload.
Additional audit. $39 for one complete audit beyond those included in your Family month. This is a one-time charge. It does not renew.
Cancelling. You can cancel anytime from your dashboard, under "Manage or cancel subscription." Cancellation takes effect at the end of the period you have paid for, and Family stays active until then.
Price changes. If we change the price of Family, we will give you notice before the change applies to you. The new price applies from your next renewal after the notice. You can cancel before then.
What each plan includes, and its price, are shown on the pricing page and at checkout. Payments are processed by Stripe.
Refund policy
We want you to be satisfied with Verity Claims™. Our refund policy is as follows:
One-time purchases (Evidence, $99; additional audit, $39). If you are not satisfied, contact us within 14 days of purchase for a full refund, provided we have not yet generated and delivered the dispute letter and evidence package for that bill. Once the evidence package has been delivered, the fee is non-refundable, because the work product has been produced.
Family plan. You may cancel at any time and will not be charged for the next billing period. Family fees already charged for the current period are non-refundable, except that if you cancel within 7 days of your first Family charge and have not generated an evidence package during that period, we will refund that first charge on request.
How to request a refund. Email support@verityclaims.co with your account email and the charge in question. We process approved refunds to your original payment method through Stripe. Nothing in this policy limits any non-waivable refund or cancellation right you have under applicable law.
Unsent letter drafts
A letter that has not been mailed is deleted after 30 days in a row without editing, saving, or approving it. We email you 7 days before and again 24 hours before. This applies to paid letters too. A deleted draft cannot be brought back. You can generate a new letter from your case, but it is a new draft, not the old one.
Mailing a letter
If your plan includes mailing, you can ask Verity Claims™ to print and mail a letter you have approved. You authorize each mailing separately, and we mail only on your instruction. The letter goes to the recipient and address you confirm, and it is sent in your name.
Mailing is carried out by a mail vendor. We show you the vendor's estimated delivery date. It is an estimate only. Verity Claims™ does not receive confirmation of delivery and does not promise that a letter will arrive, arrive by a given date, or be acted on.
You are responsible for the recipient address and for any deadline that applies to your dispute.
Deleting your account
You can ask us to delete your account and data at Delete your account and data, or by emailing support@verityclaims.co.
We will generally complete an authenticated deletion request within 45 days. When reasonably necessary, we may extend that period once for up to an additional 45 days and will explain the reason for the extension. Your account is locked while the request is open. Deletion cannot be undone.
What is kept. A record of your request. A payment record with your personal details removed. And, only if a letter was mailed for you, a record of that mailing with no health details in it, kept for up to seven years. Backup copies are removed later, as the Privacy Policy explains.
Deleting your account does not cancel a charge already made. If you have a Family subscription, cancel it first so that it does not renew.
Service changes and availability
Verity Claims™ is under continuous development. We may add, change, or remove features, and the service may be interrupted or unavailable at times. We make no service-level commitment.
Intellectual property
The Verity Claims™ audit method, scoring models, datasets, software, and brand are proprietary and confidential, and remain our property. We give you a personal, non-transferable right to use the service for your own bills while these terms are in effect. You may not copy, resell, or build a competing product from it.
Disclaimers
The service is provided "as is" and "as available," without warranties of any kind, whether express or implied, including fitness for a particular purpose and non-infringement, to the fullest extent permitted by law.
Limitation of liability
To the fullest extent permitted by law, Verity Claims™ and its operators will not be liable for any indirect, incidental, or consequential damages, or for any lost savings or recovery, arising from your use of the service. Our total liability for any claim relating to the service will not exceed the amount you paid us in the twelve months before the claim.
Nothing in these terms limits liability that cannot be limited under applicable law, including liability for fraud, willful misconduct, or a violation of law.
Your responsibility for misuse
If someone brings a claim against us because you uploaded documents you had no right to submit, or because you misused the service in breach of the "Acceptable use" section, you agree to cover the reasonable losses and costs we incur from that claim. This is the only indemnity in these terms. It does not apply to ordinary use of the service, and it does not apply to the extent we caused the loss.
Termination
You may stop using Verity Claims™ and ask us to delete your account at any time. We may suspend or end your access if you violate these terms, or when needed to protect the service or its users. Sections that by their nature should continue, such as the disclaimers, the limitation of liability, and the dispute-resolution terms, continue after these terms end.
Governing law
These terms, and any dispute arising out of or relating to them or to the service, are governed by the laws of the State of Montana and applicable U.S. federal law, without regard to conflict-of-law rules. This choice does not take away any consumer protection that the law of your home state gives you and that cannot be waived by contract.
A dispute that proceeds in court, other than in small-claims court, will be heard in the state or federal courts located in Montana.
Dispute resolution and arbitration
PLEASE READ: you are giving up the right to a jury trial and to participate in a class action. This section affects your legal rights.
Informal resolution first. Before either of us starts an arbitration or a court case, the one with the complaint will describe it in writing to the other. Write to us at support@verityclaims.co. We will write to your account email. Each of us will try in good faith to resolve it for 30 days. If it is not resolved within 30 days, either of us may proceed as set out below.
Binding individual arbitration. Except for the exceptions below, any dispute, claim, or controversy arising out of or relating to these terms or the service will be resolved by binding individual arbitration administered by the American Arbitration Association (AAA) under its Consumer Arbitration Rules, rather than in court. There is no judge or jury in arbitration, and court review of an award is limited. The Federal Arbitration Act governs this agreement to arbitrate. The arbitrator's award may be entered in any court with jurisdiction.
Fees. You pay no more than the consumer filing fee that the AAA rules set. We pay the remaining AAA fees and the arbitrator's fees where those rules require it.
Where. Any hearing will take place in the county where you live, or by video or phone, at your choice. The case may be decided on written submissions if the AAA rules allow it.
Class-action waiver. You and Verity Claims™ agree that each may bring claims against the other only in an individual capacity, and not as a plaintiff or class member in any class, collective, consolidated, or representative proceeding. The arbitrator may not consolidate more than one person's claims and may not preside over any class or representative proceeding.
Exceptions. Either of us may bring an individual claim in small-claims court if it qualifies. Either of us may go to court to seek an injunction or other equitable relief to protect intellectual property. Nothing here stops you from bringing a concern to a federal, state, or local government agency. Nothing here waives a right you cannot waive under applicable law.
30-day opt-out. You may opt out of this arbitration agreement within 30 days of first accepting these terms. Email support@verityclaims.co from your account email address, or include your account email, with a clear statement that you opt out of arbitration. Opting out does not affect any other part of these terms, and we will not treat you differently for it.
If part of this section cannot be enforced. If the class-action waiver is found unenforceable for a particular claim or remedy, that claim or remedy will be separated and heard in court, and everything else stays in arbitration. If any other part of this section is found unenforceable, the rest of it still applies.
Changes to these terms
We may update these terms. When we make a material change, we will tell you by email or in the product before it takes effect, and we will post a new "Last updated" date and version. The change applies if you keep using Verity Claims™ after its effective date. If you do not agree, stop using Verity Claims™ before that date. You can cancel Family and ask us to delete your account.
If we change the "Dispute resolution and arbitration" section, you get a fresh 30 days from the effective date to opt out of the changed arbitration terms, in the same way as above. A change to that section does not apply to a dispute we already knew about before the change took effect.
Other terms
These terms, with the policies they link to, are the whole agreement between you and us about the service. If a court finds part of these terms unenforceable, the rest still applies. If we do not enforce a term, that is not a waiver of it. You may not transfer your rights under these terms to someone else.
